Preferred Master’s-Level Responsibilities:
This role may also support advanced psychosocial assessments, interdisciplinary care planning, and discharge planning for patients and families with complex medical, emotional, and social needs. The Social Worker will collaborate closely with medical, nursing, and administrative staff; provide consultation on patient rights, community resources, and financial assistance programs; and assist with compliance with federal and state regulations. Experience or education in medical or clinical social work, program development, and quality improvement initiatives is preferred.
The Licensed Social Worker LSW oversees the planning, developing, organizing, implementing, evaluating, and directing of the Social Services Department within the facility in accordance with current existing Federal, State and local standards and works directly with discharge planners, family or responsible party to initiate tour of facility and follow up procedures.
